How much

The order of magnitude is €50 a year per person, often twice that for a child. Amounts and conditions differ from one fund to the next and from one year to the next. The form your health fund sent you is what counts.

Two ways to get it

  • You received a form from your health fund: drop it into your space. That is the best case — it already carries your membership number, your sticker and the barcode of your file.
  • You received nothing: pick your health fund from the list and the club starts from a blank form.

Not to be confused with the payment certificate

Under a paid fee, “Payment certificate (PDF)” is another document: a letter signed by the club stating that you paid, useful for an employer or a works council. Your health fund usually wants its own form, the one described on this page.

What the club fills in, and what it does not ask

The name of the non-profit, its address, the sport, the amount actually paid, the period covered, the club stamp and a signature. Your own half — membership number, sticker — stays yours. A club has no need to ask you for your national register number for this: your health fund already has it, you are one of its members.

The fee has to be paid

The form carries a sentence along the lines of “certifies that the sum of … has been paid”. A club does not sign that for a deposit. While the fee is not settled your request waits; as soon as it is, the document goes out.

How long

Fee paid, the document comes straight out of your space. Otherwise your request joins the club's queue and reaches you by email as soon as the payment is confirmed — you do not have to ask again.

One limit, said plainly

The recognised forms today cover the health funds of Wallonia, Brussels and Flanders, in French and Dutch, and Mutualia in German. An edition the tool does not know yet has a different layout: it is refused rather than filled in crookedly. That is on purpose — a document filled into the wrong boxes is rejected months later, and it is you who finds out, not the club.
